Here is an example:
A 60-year-old, Native American male presents to the emergency room with jaundice, severe abdominal pain,
nausea/vomiting. A diagnosis of pancreatitis is made. You are discussing the diagnosis and the treatment plan
with the patient, who is alone in the room. Although he is alert, he is looking away from you and is staying
quiet. When you are finished discussing things, he stays quiet for a period of time. After a long pause, he tells
you that he does not want to use the therapies you suggest and wants to wait to start therapy.
Important concepts for the acute care nurse practitioner to consider when caring for Native American patients
include:
Careful consideration is given to many situations. Give these patients time to think about the diagnosis and
treatment plan (Esposito & Kahn-John, 2020).
Do not rush education or obtaining consent (Esposito & Kahn-John, 2020).
Loudness can be interpreted as aggressiveness and may make them defensive. Use a quiet and calm voice
with interactions (Medical Staffing Solutions, 2020).
Ask them about their cultural health practices and incorporate them into treatment plan if possible (Esposito &
Kahn-John, 2020).
Extended family is extremely important and if not present, could delay care or acceptance of treatment (Huff,
2021). Allow for the entire family to be involved in care if patient requests (Huff, 2021).
Questions:
What are the potential reasons this patient does not want to undergo treatment at this time?
How would you address the refusal of treatment?
What cultural health practices do you anticipate will be important to this patient?
How would you accommodate family involvement if there is a visitor limit?
