healthcare administrative fellowship: personal statement/essay for acceptance

Note: There are two separate parts to this assignment, a personal statement, and a short essay; they shall NOT be mixed together. This writing assignment project will be submitted to a healthcare organization (Kaiser Permanente) in order to be accepted to a healthcare administrative fellowship program (after completion/graduation of master of healthcare administration (MHA) program.

Personal Statement: In your personal statement, please discuss 1) your decision to pursue a fellowship, 2) why you are interested in Kaiser Permanente as an organization, 3) the specific skills you would bring to the fellowship, and 4) your career objectives following the fellowship. Please limit your statement to 500 words or less.

Essay: Please answer ONE of the following essay prompts in 300 words or less:

1) Tell us about a time when you had a significant impact on a person, group or organization.
2) Describe a failure or setback that you experienced. What role did you play and what did you learn about yourself?
3) Describe the environment in which you were raised- your family, home, neighborhood or community- and how it influenced the person you are today.
