Clinical Experience: Pediatric Strep Throat

1. Patient Presentation and Assessment

This week, I cared for a 5-year-old male patient presenting with a chief complaint of a "sore throat" for the past 24 hours. His mother reported a fever that started last night, with a maximum temperature of 102.1∘F (38.9∘C). The patient was irritable and had difficulty swallowing, even refusing liquids. He had no cough, rhinorrhea, or body aches, and there was no known sick contact in the home or at school.

Upon physical examination, the patient appeared tired but was alert and cooperative. His throat examination revealed a very erythematous and swollen pharynx with white patches of tonsillar exudate. I also noted several small, pinpoint petechiae on the soft palate. The anterior cervical lymph nodes were bilaterally enlarged and tender to palpation. His skin examination revealed a faint, sandpaper-like rash on his trunk and neck. His heart and lung sounds were clear, and the remainder of his exam was unremarkable.
