The project report should cover but limited to the accident background, initiating and propagating events, incident details, available safeguards, failure of safeguards, consequences and lessons learnt. The report must also suggest the strategies by which the accident could have been prevented. The report must support the findings/arguments by engineering calculations, wherever applicable.
