Order Description

The purpose of this assignment is to develop an introduction and outline of the Table of Contents and source material in a reference list for the Environmental Scan of
a population-based service plan using the topic of a chronic respiratory conditions including asthma. Planning for a particular population group relates to planning
for a health issue (or issues) for a particular population cohort. This type of planning may also target a particular geographic catchment (eg the State of New South
Wales OR Western Sydney LHD etc).

Task
Prepare an Introduction and a Table of Contents for an Environmental Scan and document source material in a reference list.

Each section of the Table of Contents needs to explain the information to be included in the Environmental Scan.

The Federal Department of Health (DoH) defines the term ‘chronic conditions’ as referring to a broad range of chronic and complex health conditions across the spectrum
of illness, including mental illness, trauma, disability and genetic disorders. The Department states:

Chronic conditions have complex and multiple causes; are generally long-term and persistent, and often lead to a gradual deterioration of health and loss of
independence. While not usually immediately life threatening, chronic conditions are the most common and leading cause of premature mortality. Many chronic conditions
occur across the life cycle, although they
become more prevalent with older age and can compromise quality of life and create limitations and disability.

1) Your topic of chronic condition is chronic respiratory conditions (including asthma).
2) Choose a population base for the plan. It can be a Local Health District or State or even a national plan and should relate to the population with the condition, or
risk factors which may lead to onset of the condition.
3) Prepare an introductory section explaining the purpose of the Plan and justification for selection of the condition and population to be served.
4) Prepare an outline of your Environmental Scan including the material to be used for an overview of relevant policy and planning documents, demographic profile for
the selected population, Determinants of Health, Burden of Disease data for the selected condition. Include a list of relevant morbidity and mortality data. You can
use dot points to list relevant source material. Do not include the actual material at this stage.
5) Refer to other relevant planning documents including existing plans on the same topic. They need to be referenced if this Plan is building on other documents.
6) Include your reference list.

Criteria
Your assignment will be assessed on the following:
• Relevant range of reference material (provide at least 4 references starting only from 2013).
• Proof reading of text to ensure accuracy of grammar, spelling and punctuation.
• Writing clearly and in an appropriate formal style for an academic report.
• Ability to introduce the chosen topic and outline the sections in a Table of Contents.
