PAEDIATRIC CASE STUDY – Miss Amelia Taylor, a 4 year old female, has presented to the emergency department with an acute exacerbation of asthma. Amelia’s mother states
Amelia is very teary and fearful about hospital as she was very distressed about having her IV inserted during her last admission for asthma. Amelia has had three
previous hospital admissions for asthma in the last 6 months. When you ask Amelia’s mother about Amelia’s asthma management plan she responds “What’s that? I don’t
think she has one, we just give her Ventolin when she looks like her asthma is playing up.” Amelia’s mother has noticed Amelia has been coughing more at night recently
as the weather has become cooler and reports Amelia had a “bad cold since last week”. Amelia last had Ventolin via her spacer this morning at 6.30AM and paracetamol
300mg at 8AM. When you check the medical record, the Respiratory Nurse has documented an asthma management plan and discussion held with both parents prior to Amelia’s
discharge 6 weeks ago.
Upon closer examination you note that Amelia is a pale, teary 4 year old girl clinging to her mother. Amelia is coughing and is only responding to questions with yes
and no. Her observations are HR 136/min, RR 38/min and Temp 37.6. Oxygen saturations 90% in air. She has clear rhinorrhoea and a frequent moist cough. She has
bilateral expiratory wheezes upon auscultation and moderate sub-costal and intercostal recession with a moderate tracheal tug. Amelia’s weight is 20 Kg and her hight
is 102 cm. Her medication chart states that Amelia is prescribed the following medications for her asthma – salbutamol 6 puffs via spacer q20 minutely x 3 and then
salbutamol 6 puffs via spacer 1/24 until review – Prednisolone 20mg oral stat – oxygen 6 litres via face mask – EMLA cream 2 cm patch (applied at triage).
Amelia vomits the Prednisolone so an IV is inserted for IV Hydrocortisone orders 40mg IV q6h.
WHAT YOU NEED TO DO –

intervention, why it is suitable, any relevant positive or negative aspects/ considerations) and support/ justify the use of these interventions with reference to
current evidence-based literature. Outline the evaluation criteria/ data that would indicate that each intervention is improving or resolving the identified problem.
Conclusion (around 150 words) briefly restate the patient’s condition and problems and how the nursing interventions can benefit the patient’s health.
You need to discuss at least 1 independent nursing intervention in our essay.
